TERMS AND CONDITIONS
General Terms:

Cancellation and Rescheduling Policy:
Group Cancellations are subject to a $150 fee, individuals are subject to a $25 fee and rescheduling must be
made no later than 2 days prior to the date of the class that is scheduled. Failure to submit a rescheduling
request within 2 days will incur a $50 rescheduling fee. As per the discretion of Creative Dental
Connections LLC, exceptions will be granted to the 2-day rescheduling policy in the instance of: cases of
personal illness, course cancellations, and other exigent circumstances.
No Show Policy:
Students who fail to show for any class in person or online including the cpr hands-on skills session will not
be refunded. In the event of an emergency see the rescheduling policy. Please note: if you are not able to
attend the class and/or cpr skills session, due to an emergency you must call or Email Creative Dental
Connections LLC prior to the start of the class at: 201-320-7022 or danni@creativedentalconnections.com
Book Policy:
AHA requires “each student to have a current course textbook readily available for use” before, during and
after a course.
We sell new AHA books (BLS for $15, Heartsaver CPR/AED or First Aid/CPR/AED for $3). Add $2.50
shipping if a student wants a book mailed. If purchasing a book at the same time as enrolling for a class, we
assume you want the book brought to the class by the instructor. Alternately, you can advise us to mail it to
you (with a $2.50 shipping charge).
You can return new (unopened) books prior to the start of class for a 100% refund. Do this by returning the
book in person, or pay for shipping to us. Opened or used books may not be returned or refunded for any
reason. Books purchased for your class may not be used during the class and then returned for a refund
unless books are unopened and still in original wrapping.
Shipping Policy:
This policy applies to those books purchased that are mailed to a student’s home or work address.
Shipping policy for Website purchases: Books purchased on our website will be processed for shipping
within 3 business days.
Items are shipped only to U. S. addresses. If a shipment is returned to us because the receiving end refused
the package or is not available to accept the package, the purchaser will be responsible to pay for reshipping
and re-handling fees.
You may return purchased items “in brand new, unopened” condition within 30 days of receipt. for a full
refund minus the shipping charge. The purchaser returning the item is also responsible for the cost of shipping

Limitation of Liability & Full Release of Creative Dental Connections LLC:
Client agrees to fully release Creative Dental Connections LLC, its owners, employees, affiliates, authorized
agents, and independent contractors from any and all liability, claims, demands, or other actions that the client
may have for injuries, disability, death, or other damages of any kind, including but not limited to direct,
incidental, indirect, punitive, or consequential damages, whether arising in tort, contract, or breach
warranty, arising out of participation in the services, including, but not limited to, the physical activities of
the CPR training even if caused by the negligence or fault of Creative Dental Connections LLC, it’s owners,
employees, affiliates, authorized agents, or independent contractors.
Privacy Policy:
Creative Dental Connections LLC is committed to keeping any and all personal information collected of
those individuals that visit our website or make use of our online services secure, confidential, and private.
Our privacy policy has been designed and created to ensure those affiliated with Creative Dental
Connections LLC of our commitment and realization of our obligation not only to meet but to exceed most
